Complete Family with twice IVF: Two Boys and Two Girls

Puan Lina, 32 years old and Encik Muhzan, 39 years old from Muar, Johor were married for 5 years and had been trying to have at least a baby of their own. They had tried traditional medicine without success.

Following friend’s recommendation, they came to see the fertility specialist for advises. Puan Lina’s detailed investigations showed that her right tube was blocked. Laporascopy was done and she was found to have endometriosis which was treated surgically.

On 1st July 2011, she started her injection for her long IVF protocol. Egg pick up was performed 20 days later and three days later, embryo transfer was performed with 3 embryos at 7, 8 and 9-cell stage. Surprisingly, her serum β-hCG test was 386 IU/L on day 12 post embryo transfer. Urine pregnancy test was positive. Six weeks later, two pregnancy sacs with 2 fetal hearts were seen during ultrasound scan. She has twin! She delivered her twin on 29th March 2012 by Caesarean Section. Both were baby girls weighing 2.43 kg and 2.65 kg and are well and healthy.

Two years later, she wants to have her third baby. In December 2014, she had her second IVF treatment at Metro. Three embryos, at 4-cell stage were transferred two days after her egg pick up. Three weeks later, two gestational sacs with fetal hearts were seen. She had twin pregnancy again! At 20th week gestation, the detailed scan showed that she had 2 healthy boys. Wow! Two boys this cycle! Her pregnancy progressed well and she delivered 2 healthy boys by Caesarean section. She and her husband were very happy to have completed their family with Metro IVF and have two boys and two girls of their own.

Thank you Metro IVF.

All Good Things Come To Those Who Wait

Subramaniam and Louisana are nice couple from Kapar town. They were married in 1999. In their quest to complete their family, they tried hard to get pregnant naturally but remained childless after 15 years of marriage. In 2012, Lousiana was already 41 years old and the hubby was a decade more senior than her. Then, they decided to try medical treatment.

Between 2012 and 2013, they had attempted a total of six IUI cycles at several centres but were all futile.

Despite their many failures, they did not yield. Then, they were recommended by friends to come to Metro IVF in August 2013. Upon a detailed investigation and consultation, the fertility specialist , Dr. Tee advised Louisana to go for IVF with ICSI treatment. She agreed to the treatment and scheduled their first IVF cycle in May 2014. She had her eggs pick up on 9th June 2014 and three embryos were transferred back to her wombs two days later. Her pregnancy progressed to 7th week. To her disappointment, this first cycle ended with early miscarriage.

With the support and encouragement from her husband and the IVF counselor, she remained optimistic and regained her confidence soon. She accepted a new challenge and went for the second cycle with Metro IVF six months later. This time, the Fertility Specialist picked up her eggs on 8th December 2014 and successfully transferred two embryos back into her womb two days later. Although the embryos were of Grade 3, the cycle was successful. Her urine test was positive and her Beta-hCG was 139 IU/L twelve days after the transfer. Her pregnancy progressed normally. She delivered a healthy baby boy on 18th August 2015 by Caesarean section. The couple was so happy that after waiting for 15 years, they finally have a baby boy of their own.

All good things come to those who wait.

Too Late For A Miracle???

Mr and Mrs LML are a lovely couple from Singapore. They got married at 40 years old and have been trying for a child since. They had 3 failed IVF cycles and their fertility specialist in Singapore did not recommend her to try again with her own eggs as her FSH level was already very high. Her AMH levels (ovarian reserve indicator) were low which confirmed that she was nearing menopause.

Mrs LML went to numerous centres for a second opinion but decided to finally settle for Metro IVF as she had read and heard many good reviews. She finally decided to have an IVF with Egg Donor cycle with Metro IVF as she was very comfortable with the doctor and staff here.

Mrs LML commenced her cycle in March 2016. Her donor produced 16 mature oocytes which was then matched with her husband’s sperm via ICSI. 2 Grade 2 and 1 Grade 3 embryos were transferred on ET day and 3 embryos were frozen for future use.

In early April 2016, Mrs LML informed us that her BHCG was 116.4IU/L. When she repeated BHCG after another 4 days, it increased to 402IU/L. We advised her to go for a scan to detect the gestational sac and heartbeat. Unfortunately fetal heartbeat was continuously not detected till 8 weeks and D+C was scheduled.

Mrs LML was advised to rest for a few months before attempting FET with her remaining embryos.

In September 2016, Mrs LML started her FET cycle. FET was done with 2 Grade 2 and 1 Grade 3 embryos.

2 weeks later, she informed that she was pregnant again. Her BHCG was 238.8IU/L and progressed to 1355.2IU/L 4 days later.

Their family life is finally complete when she delivered her baby boy  in June 2017.

A Pair Of Libyan Angels: Two Is Better Than One!

Madam Fama and her husband are from Libya and were married in 2010. They are expatriates currently residing in Pahang and came to us in 2013 seeking treatment for infertility following recommendations from their friends. Madam Fama attempted to try for pregnancy a few times with Clomid as she was wary of doing too many injections during IVF. Her previous experience of doing IUIs in Pakistan also made her wary as she was uncomfortable being probed and scanned as she was a bit shy in nature.

Two years later, after numerous unsuccessful tries with Clomid, we finally managed to persuade her to reconsider IVF. Her IVF cycle commenced in August 2015. 10 oocytes were harvested and ET with 3 embryos was done 3 days later.

Her luteal support appointment was uneventful when she was suddenly admitted to ward as she was displaying symptoms of OHSS. Blood was taken to confirm OHSS and to check for her BHCG level. Her BHCG was 373IU/L which indicated that she was pregnant and it was probable that her pregnancy might have aggravated her mild OHSS symptoms.

Initially, one gestational sac was detected but as her pregnancy went along, we detected two sacs with heartbeats. Her pregnancy progressed well and further scans showed that she was carrying twin girls!

She has recently delivered and the couple is happily enjoying the presence of their little girls..

A Tough Road To Pregnancy

Mrs Yi and her husband are from China. Her husband works for an international company and his work led them to relocate to Malaysia. They have been actively trying to get pregnant since they got married way back in 2010. Unfortunately, the long wait has never yielded any positive result. They finally decided to go for fertility testing in Metro IVF when Mrs Yi came numerous across encouraging reviews online. Semen analysis indicated that Mr Yi has azoospermia and further testing revealed that he was also not able to produce any sperm. Mr and Mrs Yi was devastated at first but her ever supportive husband was open to the idea of having a sperm donor and encouraged Mrs Yi to try.

Mrs Yi underwent IUI in April 2014 and June 2014 but pregnancy eluded them.

They finally opted for IVF in August 2014. On OPU day, 11 oocytes were harvested but only 3 were good for transfer on ET day. Sadly, these embryos failed to implant.

Though both of them felt very disappointed and very disheartened, they still wanted to try again for a child. They were advised to rest for a few months and they finally came back in April 2015 to try IVF again. This time around 17 oocytes were harvested, 3 embryos were transferred and another 3 embryo were able to be frozen for future use.

2 weeks later (May 2015), when the UPT strip showed a double line and her pregnancy was confirmed with a BHCG of 953IU, Mr and Mrs Yi was overwhelmed with emotion. They were so happy and the joy was doubled when they saw 2 gestational sacs with heartbeat (and 1 smaller empty sac) a few weeks later. Mrs Yi made preparations to go back to China as her visa was due and she wanted to be with her family who could take care of her.

Events took a drastic turn in September 2015 when Mr Yi made a call to inform us that Mrs Yi had miscarried her twins at 25+weeks due to cervical imcompetence. He indicated his wife’s desire to come back for an FET cycle with their remaining embryos. Mrs Yi was advised to rest for at least 6months to recuperate.

Nearly a year later, in August 2016 Mrs Yi came back to us to start her FET cycle. Her steely determination and bravery left us in awe. 3 embryos were transferred in September 2016 and when she emailed from China indicating that she was pregnant again, all of us were overjoyed. She is currently a stay at home mom to her precious baby girl!! Her steely determination has finally been rewarded.

Oldest IVF Mother Using Her Own Egg in Malaysia at Age 46

Getting Pregnant Over The Age of 40

A woman is born with all the eggs she will ever have in her lifetime. Unlike a man’s gamete, the sperm which can be regenerated, the woman’s egg reserve is finite and decreases as she ages. Apart from the lower number of egg reserve, the eggs will also age which results in the quality of the egg to decline. This is why women at older age are at risk of miscarriage and genetically abnormal child births. Statistically, a woman has less than 5% chance of getting pregnant when she reaches 40, and almost zero as she approaches menopause (no more eggs).

Is It Possible To Conceive Over 40 Using Own Eggs?

There are 3 known cases worldwide, where women are able to conceive using their own eggs at the maximum age of 46 in Fertility Care Orlando USA, Create Fertility London UK, and Metro IVF Malaysia. With the help of IVF, pregnancy can be achieved.

It was in 2012 when Pn. Lina, a local Malaysian, came to Metro IVF for her first fertility visit. She planned to get pregnant when she was 45. Physical and medical examination showed she had poor ovarian reserve and it would take a miracle for the IVF laboratory to achieve pregnancy for her. During the consultation, she was also informed by the consultant and the fertility counsellor that her chance of getting pregnant is less than 1%. Furthermore there is increased risk of miscarriage of over 20%, risk of abnormal baby and also the risk of hypertention, diabetes, and other medical illneses. She decided to seek other fertility opinions. In 2013, Pn Lina came back and requested to proceed with IVF treatment. She was further counselled on the low chance of achieving pregnancy and other risks. The fertility team worked hard and together with the embryologist and andrologist had a long discussion before proceeding with the case. It was decided that the best stimulation protocol for her would be short IVF protocol (Flare-up) She had 2 follicles and 1 egg. ICSI was done for the single egg to ensure fertilisation. The inseminated egg was then cultured in a mint incubator with mixed gasses and media to ensure optimal progress. The following day, fertilisation was confirmed by the embryologist that there was 2 pro-nucleus and 2 polar bodies. The team knew they have made the right decision.

On the second day, the embryo had divided into 4 cells. The fertility team collectively decided to transfer the 4 cell embryo instead of culturing it further (Although the standard procedure requires cultivation to 3 days or 5 days). In-vitro is better than in-vivo. The embryo transfer was then performed by the most senior and experienced clinician and senior embryologist. The embryo transfer went through smoothly and was placed 1cm from the fundus. The team was relieved as the embryo was placed at the best position. After the transfer, it was decided that Lina should have adequate luteal support to ensure implantation. She was given both progesterone injection and vaginal progesterone. During the three weeks that follow, she was regularly supported by the team of fertility counsellors. 3 weeks later, Pn. Lina was confirmed pregnant and a healthy baby was delivered at term via lower segment cesarean section (LSCS) .

At last, the team of clinicians, embryologists, counsellors at Metro IVF were delighted that all their hard work and time spent was not in vain. It would have taken a few hundred cycles on average to achieve 1 such pregnancy. To date, this is the oldest successful IVF woman to conceive and deliver a healthy baby using her own egg in Malaysia.

This inspirational story of Pn. Lina was also featured on The Star news paper. Read more here.
